my teeth have a constant ache.help?

all of my top teeth ache constantly this has been going on for several years…its a dull pain radiating from the gums up…i visit a dentist regularly(but never ask about it mostly because i **** being at the dentist and my mind goes to other places im also quite embarrassed of my teeth and do not like talking about them) i also brush on a daily basis but i always have cavities…they are also very sensitive to cold and hot sensations…im not sure whats wrong i just want to know how i cant stop it…it can become so bad that it gives me horrible head aches….i also have an underbite(although isnt that noticeable unless your looking in my mouth or i smile(which i avoid doing), i also have a slight cross bite…i have tried braces but the pain was so great i could not even get out of bed and had to have them taken off… the bottom teeth do not hurt like the top do but i do have the same pain in my jaw where it connects to my skull(not sure how to put that) please help..
